Otherwise contact Shazam Customer Service at 800-383-8000 and use the following options when prompted:
* Press #3 - for all other inquiries
* Press #3 - since your are a as a Cardholder
* Press #1 - to report your card lost or stolen
* A Shazam representative will take your information, deactivate your card
and contact us at First State Bank the next business day
